Services
PACE Centers are responsible for the delivery of all primary, acute and long-term care, as well as all appropriate social, restorative and supportive services. Care managers evaluate each participant’s needs and then develop individualized care plans that may include:
- Adult day care
- Emergency services
- Home care
- Hospital care
- Meals and nutritional counseling
- Medical equipment
- Physical, occupational and speech therapies
- Prescription drugs
- Primary and specialty medical care
- Recreational therapy
- Transportation
- Vision care
