Case managers and nurses work together to identify the services needed to help families keep their loved ones living at home. They assess each participant's unique needs and then coordinate a comprehensive care plan that provides services that may include:

  • Adult day care and caregiver respite
  • Caregiver training and support groups
  • Companionship
  • Emergency alert response systems
  • Housework and chores
  • Home-delivered meals
  • Home modifications
  • In-home counseling
  • Medical supplies and equipment
  • Nursing services
  • Personal care, such as eating, bathing, dressing, bathing and toileting
