For 25 years, the Channeling Project has helped Seniors who are eligible for Medicaid continue to live independently in their own homes. The Business Journal of South Florida has recognized the Miami Jewish Health Systems Channeling Project with the “Excellence in Healthcare for Nonprofit Community Based Programs” award.
The project makes sure that you or your loved one receives regular medical care and attention—in fact, it’s a requirement. Care managers determine the unique needs of each participant, develop a plan of care to meet those needs and then coordinate services designed to help the participant live at home independently.
The project serves people living in Miami-Dade or Broward counties, age 60 or older, who are eligible for care in a nursing home as required by Medicaid. Miami Jewish Health Systems operates the project on behalf of the State of Florida's Agency for Health Care Administration and the Department of Elder Affairs.
